Before You Apply

Please review the checklist of information before you start your application, so you can be fully prepared. Additionally, it’s important to read the instructions found here.

Important note: The 2019-2020 application has been revamped for this year’s cycle to make the user experience easier and more efficient. Please make sure you are answering each question very carefully. Click on the link above (instructions) to preview the changes to the application. Below are some of the changes you may notice.
The application will open Friday, February 1, 2019 at 9:00 AM eastern and will close Monday, April 1, 2019 at 6:00 PM eastern.

The new application features the following changes:

  1. Pre-qualification questions to guide applicants to the correct application or discontinue an application if an applicant does not qualify for the Folds of Honor Scholarship;
  2. Shorter application if an applicant has previously been awarded a scholarship with Folds of Honor;
  3. Some questions have been eliminated and some have been combined to shorten the application;
  4. Logical questions/options will appear or disappear depending on your responses to the previous questions
  5. ONLY the required documentation described in the Checklists will be accepted!

Checklist of required information for the Children’s Fund Application

Collecting this information will help you complete the online application. We do not accept any additional information, including personal information, documentation, email changes, or any other items outside of the online application. Any item mailed, emailed, or faxed to Folds of Honor will be recycled.

Service Member Information

  • Proof of service document – most current/final DD214/DD1300 used to verify full name, branch, rank, years of service, decorations and character of service; Guard or Reserves may not reflect most current service (an additional upload bar is available to upload the service member’s military ID to verify current rank).
    • Purple Heart documentation – required if wounded in action classification is selected.
    • Amputee documentation – required if loss of limb classification is selected; must be a medical document; must be a major limb (hand, arm, foot, leg).
  • Proof of disability document – current year VA Benefits Summary Letter that states combined service-connect disability evaluation; will not use the compensation rating/percentage. (If service member is KIA or KILD, the DD1300 can be used as disability document).

Applicant Information

  • Proof of dependency document – Military ID (front AND back, preferred) or State-Issued Birth Certificate (if under age 10); used to establish dependency upon the service member named in the application (service member must match sponsor on ID or be listed on official birth certificate), also used to verify name, dob and gender. *If applicant is a step-child the State-Issued Marriage Certificate must be uploaded. *If applicant is an adopted child legal, court-issued adoption papers must be uploaded. If applicant is a legal ward court-issued, guardianship papers must be uploaded.

School Information

  • Proof of enrollment document (private and online) – provided to you by the school on letterhead
  • Invoice or estimate (tutoring) – must list subject(s), hours, and rate; provided to you by the institution
  • Name, mailing address (found on website), phone number, website, contact email; used to vet (approve) the school.

Financial Need Information

  • Close estimate of charges for the upcoming academic year.

Essays

  • There will be 3 short essays to complete. Please provide as much information as you are comfortable sharing, as we use these throughout the year. Can be completed by the parent/guardian with input from the applicant.
